MedPath Direct provides simple but absolute processes for our agents to review and understand their commission level, contracting and product training through one electronic process. You will be required to provide your National Producer Number. If you do not know your NPN simply click on here, input your information and record your number for future reference.

Upon completion of the on-line process you are provided a unique agent ID and a personalized MedPath Direct web site. Remember to print this information for your records.

Effective Date of Insurance Benefits: If your client enrolled between the 1st and the 15th of the month, your client’s first effective date will be the 1st of the following month. Who is Eligible for Coverage?
The MedPath Direct plans were created to help the forty-seven million Americans who do not have health coverage today. Through our partnership with the Consumer Health Choice Association (CHCA), we are able to offer these plans to individuals and working people across the country. The MedPath Direct plans are guaranteed issue, so everyone qualifies regardless of pre-existing medical conditions. All eligible applicants pay the same rate provided you are under age 65 upon enrollment.*

These plans were designed primarily for people who:

• Don’t have access to comprehensive major medical insurance
• Cannot afford or qualify for traditional major medical insurance
• Have catastrophic health insurance plans with very high deductibles

*Primary member and spouse (if applicable) must be at least age 19 and under age 65.

The AMLI Limited Benefit Medical Plan is a group insurance benefit program. The group insurance benefits vary depending on the plan selected. These benefits are provided under the group insurance policy underwritten by the American Medical and Life Insurance Company (Policy Form No. AMLI GRP LM 2007) and is subject to the company’s underwriting guidelines, exclusions, limitations, terms and conditions of coverage as set forth in the insurance policy. This insurance is not basic health insurance or major medical coverage and is not designated as a substitute for basic health insurance or major medical coverage. This is a limited medical plan that provides for limitations to the coverage for each benefit. In some circumstances benefits provided will vary as required by state law and the plan may not be available in all states. The insurer has the right to increase premium rates and has the option to cancel coverage. These plans are offered through CHCA (Consumer Health Choice Association) and require membership in the association. The association fee as well as the costs for the other benefits will be included in the monthly rates.

Insurance Based Features “Free-Look” Period
You are given a ten (10) day “Free-Look” period to review your General Membership program and cancel with a full refund of your initial monthly payment, first monthly association fee and one-time processing fee. Exceptions: Residents of AZ and CO are given a thirty (30) day “Free-Look” period. The first day of the “Free-Look” Period shall be determined using the date of your application, plus seven (7) days - allowing for printing and standard mail delivery. A cancellation within the “Free-Look” Period is determined if the date stamped on the cancellation request falls within 17 days (37 days in AZ and CO) of the date of your application.

What is MedPath Direct Health?
MedPath Direct Health is a membership program consisting of two components – health insurance benefits and membership savings features.

Let’s first discuss what the health insurance component of the program is?

The health insurance portion of the program is a limited indemnity medical insurance program. Your benefit is a fixed dollar amount for specific health related occurrences such as hospitalization, doctor’s visits, or surgeries and more. For example, let’s assume you are hospitalized and your program choice is the MedPath Premier. Your benefit will be $1000 a day up to 100 days per person.

The second program component is the member discount features available for health related savings on prescription, dental, vision, hearing, and more. Unlike the limited medical insurance benefits the discount features are not insurance. But our goal remains the same – helping you reduce your out-of-pocket costs when we can and where we can!

In addition to the Premier plan there are 3 additional options to choose from; the Platinum, Gold and Silver as a lower cost alternative to the initial.
